*Privacy notice – This is a Google form.  All information will be stored on a Google Server in the United States of America and thus subject to their privacy laws. Currently, Donna Fry and Mark Carbone are the only people with access to the data stored in this form, but we reserve the right to share the data with members of the #OSSEMOOC Planning committee (the facilitators). We, and the planning committee, will use this information strictly for the facilitation of #OSSEMOOC or as anonymous data in research. We will not sell or share personal information with outside entities.
